Urban Wellness is a clinician-focused, woman, neurodivergent (AuDHD), lesbian owned group private practice in Chicago on a mission to eliminate mental health stigma by creating awareness of, appreciation for, and acceptance of mental health self-care. Office Locations:
We have four office locations in various Chicago neighborhoods; Edison Park, Sauganash, Old Irving Park, and Forest Park.

What You Should Know About the Job:
Key Rsponsibilities -
- Perform on-site clinical counseling services to patients of Urban Wellness, as scheduled, and in accordance with company policies.
- Conduct and regulate counseling services in a professional manner so as to maintain and increase the good will and reputation of Urban Wellness.
- Abide by all laws, rules, regulations, and codes of ethics that are binding upon or applicable to the services performed for Urban Wellness.
- Respond to patients’ requests for service or calls within established timeframes whether such requests are made in person, by phone, voicemail, etc. or through the Urban Wellness website.
- Respond to such requests by meeting, evaluating, and providing services to such patients.
- Complete required documentation. Timely complete written records for each patient including, but not limited to: intake notes, progress notes, treatment plans, termination notes, contract notes, and other forms or documents which may be needed or required from time-to-time by Urban Wellness or third parties in conjunction with the treatment of the patient within a timely fashion (by Friday of each week).
- Maintain accurate, up to date patient files, records, plans, and reports.
- Communicate and collaborate with a patient’s treatment team (i.e., PCP, psychiatrist, school, etc.) as necessary.
- Charge and collect payments from patients for services provided consistent with the policy and rate for such services as established by Urban Wellness.
- It is a 30 hour work week with 25 of those hours dedicated to clients
- Either 2 evenings 4-8pm or 1 evening and 1 weekend
- Up to 2 days in office
- Strong working knowledge of human behavior and performance; individual differences in ability, personality, and interests; learning and motivation; psychological research methods; and the assessment and treatment of behavioral and affective disorders.
- Strong working knowledge of principles, methods, and procedures for diagnosis, treatment, and rehabilitation of physical and mental dysfunctions, and for career counseling and guidance.
- Strong working knowledge of group behavior and dynamics, societal trends and influences, human migrations, ethnicity, and cultures and their history and origins.
- Team player. Collaborate well with team members in support of our clients and community.

Minimum Required Education & Experience:
Master’s Degree or Doctoral Degree in counseling or social work; and
Active, unrestricted license (PhD, PsyD, LCSW, LMFT or LCPC) in the State of Illinois
(must be fully licensed). A copy must be provided to Urban Wellness.
At least three years’ experience post-unrestricted licensure.
Have at least one niche market for counseling services (i.e., children, teens, trauma,
anxiety, grief, anger management, etc.).
Current approval as an in-network provider with BCBS (preferred)

Work Location:

Scheduling Requirements:
Be available for client sessions work (2) evenings per week or one evening and one weekend (6 hours) per week. Note: Evenings defined as working until 8:00 pm.
Attend occasional mandatory staff meetings.
